[ditty id=11117] The following “roadmap” was created by Cornerstone Whole Healthcare Organization, INC (C-WHO) in partnership with Valor Health and the Pride in Idaho Care Neighborhoods (PiICN) Consortium with funding from the Cambia Foundation to support other communities across the state and country in developing more inclusive, safe, and culturally appropriate care environments for all marginalized rural community members. Our model demonstration happens to focus on LGBTQ+ patients and neighbors. However, we want to emphasize that this model is designed to be used with other communities within This may include racial/ethnic minorities, migrant farm-workers, individuals with serious mental illness, religious minorities, and others. The roadmap is intended to present lessons learned, emerging best practices, and resources discovered in the development and implementation of the PiICN model. Our hope is that this information can accelerate the creation of similar neighborhoods of care based on our successes and challenges. Project Resources
The Pride in Idaho Care Neighborhoods (PiICN) is generously funded through the Cambia Health Foundation. C-WHO has partnered with Valor Health to pilot a model of coordinated and whole person care to marginalized patient populations in rural areas. The demonstration is based on the question, “How can we deliver appropriate, inclusive, and safe care together with our partners?”. The findings of the project will be scaled to serve as a roadmap for other communities across the country.
